SYSO requests that students inform the appropriate Orchestra Secretary if they are going to be late to, or will miss a rehearsal by filling out an excuse form. You may either:

• Pick up an Absence Excuse Form at rehearsal. Fill it out and hand it to your secretary at rehearsal, OR
• Fill out and submit the form below.
